Avandametâ : rosiglitazone-metformin fix combination as a tool of oral antidiabetic therapy

 

 

Summary

 

Avandametâ, a fixed combination of rosiglitazone and metformin with three different dosages has recently been available in Hungary. The review article summarizes the mode of action of each of the compounds, their effects on the carbohydrate and lipid metabolism as well as their antiatherogenic „pleiotropic” properties, pointing out the advantages of their combined application. At present its use is indicated as a second choice, if maximal or submaximal doses of metformin monotherapy are not sufficient to achieve the desired glycemic control, but in the future, depending on the clinical experiences, safety, tolerability and effectiveness, it may be even wider.

Summary

Atopic dermatitis in childhood

 

The atopic dermatitis is one of the most common skin condition in children. Therefore, it is important that atopic dermatitis be included in the differential diagnosis of any infant with a pruritic dermatitis. This suspicion should be augmented if there is a family history of atopic dermatitis, allergic rhinitis, or asthma. Once atopic dermatitis has been diagnosed, the patient's dry skin must be treated, irritants to the skin must be avoided, and  topical  anti-inflammatory agents and systemic antihistamines should be used to break the scratch-itch cycle and to control exacerbations of eczema.  The traditional mainstay of topical treatment of atopic dermatitis are the steroid ointments. The new immunomodulators alter the local immune response. The balneo- phototherapy and psychotherapy plays a very important role in the treatment of atopic dermatitis. It has a great importance, for the patient, to know the therapeutic possibilities, the course of the disease. The good compliance is essential.

Sleep disturbances and performance changes in night shift workers.

Sleep and the circadian clock

Summary

 

The human body naturally follows a 24-hour cycle of wakefullness and sleepiness that is regulated an internal circadian clock. The shiftwork disturbs the rhythm of the circadian clock.  Lack of sleep, irritability, failed memory-function, and bad mood are the consequence of this malfunction with subjective and objective complaints. Shift workers perform critical functions in hospitals, on police forces , in the transportation and manufacturing industries.  So, it’s very important to solve their insomnia. To ensure a good sleep it’s needed to keep the steps of sleep-hygiene, and required the support of the environment, and if it necessary to use medical therapy as hypnotics.
